FDA Launches Rare Disease Framework Initiative for Drug Development

1 min read

The Food and Drug Administration announced the Rare Disease Framework Initiative on October 14, 2026. The initiative will develop an optimized, fit-for-purpose framework for rare disease drug and biologics development, according to the agency.

The agency said it will host an October 28 meeting at its White Oak campus to solicit feedback from patients, disease advocates, and industry. The meeting will be facilitated by the Reagan-Udall Foundation for the FDA.

The agency noted that only about 5 percent of rare conditions have an FDA-approved treatment. The initiative is led by the FDA's Rare Disease Innovation Hub in partnership with the Office of the Commissioner, the Center for Biologics Evaluation and Research, and the Center for Drug Evaluation and Research.

HHS Secretary Robert F. Kennedy Jr. said Americans with rare diseases cannot afford a regulatory system that treats every condition the same. Acting FDA Commissioner Kyle Diamantas said the agency will build a more consistent, predictable, and innovative approach to rare disease drug development while maintaining rigorous standards.

The agency said it is committed to providing greater clarity in regulatory decision-making, embedding the patient voice in drug development, using external expertise, building training resources, applying risk-based approaches, and supporting advanced science, novel methods and innovative statistical approaches.

The agency said further opportunities for input will include sector-specific roundtables, Requests for Information, and internal feedback sessions. This feedback will inform the development of a new framework including potential guidance, revisions to regulations, staff training, and organizational changes.
